Notice to All Full-Time Employees: 1095-C Forms Distribution Change

In compliance with the recent Paperwork Burden Reduction Act (PBRA) passed by Congress, there have been important changes to the distribution of 1095-C forms. Please take note of the following updates regarding the availability of your 1095-C form for the 2024 tax year.

 

What is the 1095-C Form and why is it important?

The 1095-C form provides information about the health insurance coverage offered to you by Lehigh University during the calendar year. This form is used to report whether you and your dependents were offered minimum essential health coverage, as required under the Affordable Care Act (ACA). You may need this form when filing your taxes, particularly to demonstrate that you were covered by health insurance or to determine whether you qualify for any exemptions.

 

New Process for Receiving 1095-C Forms

As part of the Paperwork Reduction Act, we are no longer automatically mailing 1095-C forms to employees. Instead, if you need a copy of your form, you may request one from Human Resources by following the directions below.

 

How to Request Your 1095-C Form:

If you would like to receive a copy of your 1095-C Form for the 2024 tax year, please complete this request form and submit it to the HR department. Upon receipt of your request, we will provide the 1095-C form by the later of January 31, 2025 or within 30 days of your request. Please ensure that you submit your request in a timely manner to allow for adequate processing time.
