Follow the links below to complete two crucial requirements to comply with the Protection of Minors Policy.

 

Program Registration Form

All programs occuring on Lehigh's campus involving minors must be registered with the university. To complete your program registration, follow this link to the required Docusign form. 
 

Parent/Guardian Release Form

All minors participating in your program must submit a completed release form signed by their parent or legal guardian. These forms must be kept in your records for no less than six years. Download the form at the link above and distribute it during your sign up process for your program.
